Becoming a member is simple.
Get a Referral
Your doctor, therapist, case manager or other care provider can give you a referral. Send them to the Providers Page where they’ll find what they need. You can also print out a referral form here. Remember, the form must be emailed or faxed to us directly from the referring clinician.
Take a tour
Once we receive your referral form, we will call you to set up an orientation time. We just need a few forms completed so we know how to best support you while you’re in the building and enjoying Choices activities. We’ll give you a tour and help fill out your registration packet.
Join the family
We really do feel like an extended family. Choices members and staff (many of whom are members themselves!) make it a priority to help you feel welcome and included in our many activities and workshops. Members are welcome to participate as much, or as little as they’d like!
